Welcome to Elmhaven East!
Elmhaven East is a 58 bed skilled nursing and rehabilitation facility, certified for Medicare and Medicaid and licensed by the Kansas Department of Health and Environment. We offer rehabilitation services for short or long term care. Respite and daycare services are also available. We encourage our residents to be active in making decisions regarding their medical needs and daily care. In addition a quarterly care plan meeting is held for each resident and family to attend. State surveyors visit our facilities annually. They look at resident care and review records. Their findings are for public viewing. We will be happy to provide you with the State of Kansas Ombudsman phone number and/or other regulatory agencies upon request.

Meal times at Elmhaven provide more than just tasty food and good nutrition it also provides social interaction and time to develop new friendships. We always encourage our residents to give us their input, so we can cook as close like home as possible. Our residents have choice with meats and vegetables, kinds of hot or cold cereal, to how they like their eggs cooked. We offer a variety of breads, deserts and even coffee cakes just to mention a few choices. Our residents consider our facility to have excellent dining and food service. They especially enjoy the soup and salad bar, scrumptious dessert trays even Belgium waffles for breakfast.
Develop new Friendships
Our residents have a monthly “residents council” meeting, discussion includes any concerns, events or ideas that residents would like to bring up. One example, the residents requested a salad bar, which they truly enjoy. Elmhaven East has many low care residents which provide a very enjoyable environment.
Special Care
Our nurses specialize in residents who require difficult medical care. We work daily with people who suffer from diabetes, respiratory problems, dementia, gastro esophageal dysfunction, LG tubes and a variety of other issues on a daily basis. Every day we work with doctors and residents families to best meet individual needs. We also have developed good working relationships with outside companies such as dialysis, wound care, cancer treatment (such as chemotherapy and radiation therapy) and hospices. Our supervisory staff has more than 159 years combined, of long term care experience. Our goal is to help residents maintain their highest level of function.
Teamwork
We apply a team approach with all of our departments, dietary, social services, activities and nursing all work together toward one goal for each resident. We hold weekly care planning meetings with residents and family members. We are proud of receiving an Exemplary Care Award in 2008. You will find that we have an “open door policy” when you need us, as well as easy access to our staff. We also hold quarterly quality assurance meetings, which include all departments and our medical director Dr. Neil Goodloe.
Cozy and Relaxed
Please feel free to customize your room to suite your style and décor (paint, furniture, decorations). All residents’ rooms have individual heat/air controls; this allows you to set the temperature to your comfort level. Many of our private rooms have recently had wood laminate floors installed, giving your room a very home like feeling. Eight of our private rooms include a personal shower. We have a family room which is available for gatherings, meals or special needs. As well as several other large areas for residents and family to gather and spend time together. One of the resident’s favorite places is our covered porch and patio. It’s a shady place to sit and enjoy many beautiful plants and flowers.
Transportation Services Available
One of the services offered is group or individual transportation for our clients at no extra charge. Transportation is available to medical appointments, luncheons, shopping trips, parades, entertainment and community events.
Our transportation fleet includes one bus, one vehicle and three vans. Our bus can accommodate 12 to 14 individuals and two wheelchair passengers. The vans are walk-in wheelchair accessible which accommodates one wheelchair and up to three passengers. We also offer one vehicle for individual transportation needs. We encourage families to accompany their loved ones to appointments.
